How do I disclose a wrongdoing?

Disclosures of wrongdoing must be in writing and to fulfill that requirement, the Office of the Public Interest Disclosure Commissioner (OPIDC) has developed the Disclosure of Wrongdoing Form. Contact our office for a copy of this form then fax, mail or deliver in person to the OPIDC; do not email. If you have any questions while completing the form, you may seek advice or call us.


How do I make a reprisal complaint?

Complaints of reprisal must be in writing and to fulfill that requirement, the OIPDC has developed the Reprisal Complaint Form. Complete this form and fax, mail or deliver in person to the OPIDC. If you have any questions while completing the form, you may seek advice or call us.


How do I request advice?

Prior to disclosing a wrongdoing, it is recommended that  you seek advice. You can seek advice from a supervisor, the designated officer of your public entity, if one exists, or the PIDC. This service is free and confidential. Please contact our office to receive a Request for Advice Form.
